Certification of Initial Disclosures is a signed/notarized form each spouse files with the court promising they provided mandatory financial disclosures to the other spouse. These certifications are used in child support, alimony, and/or equitable distribution cases. Each county courthouse has their own certification forms and their own requirements of what financial records must be disclosed. The mandatory disclosures and certification form used usually depends on whether child support, alimony, and/or equitable distribution are at issue.

Each individual county courthouse uses its own certification of initial disclosures forms. The following list is incomplete, but Mr. King will try to add to it over time:
