Affidavit of Judicial Assignment
Summary:A document used to assign a judge and schedule hearings

An Affidavit of Judicial Assignment is a form used to appoint a judge to your case and schedule hearings on the core claims. It is used at the start of litigation with the initial filings that start the lawsuit. Each individual courthouse usually has their own Affidavit of Judicial Assignment form. A lot of the form is completed by the court clerk when filed.
